1. Join the Waitlist
Call or send us an email to be added to our waitlist. You will receive a confirmation that you have been added and a team member will be in touch to offer more information.
2. Meet with your provider
A meet and greet can be scheduled where you can learn about your future provider and inform us how we can best serve you.
3. Choose your Membership
New patients join the practice through a selected membership type. These are annual commitments with unique benefits.
4. Success
Once your spot has been offered and accepted, we begin requesting records and welcoming you to the practice!

How do I become a patient?
What to Know
There are three key elements to know about becoming a patient of our practice: All patients of our practice join through one of our available memberships. Medical visits are still billed through your insurance. Every patient receives ALL our standard of care elements. Each program offers unique benefits and they are annual commitments.
Standard of Medical Care
As with all of our memberships, the standard of care you receive from our providers is best in class. None of our memberships differentiate on the quality or timeliness of care from your provider. Instead, these memberships focus on offering additional benefits in support of personalizing your care.

Meet Providers
We have a team of providers dedicated to putting you on the best path to health.
Nicole Altorelli, MD focused her medical education on innovative practices and exploring solutions for the future of integrated primary care. She completed residency at Yale University and medical school at the University of Rochester.
Alphonse Altorelli, MD has been practicing in the Washington and New Preston area for over 35 years. A pillar of the medical community, he is revered for his diagnostic aptitude, emphasis on listening and spending time with patients.
Dr. Abra Mabasa focused her medical education on international public health. She completed her residency at Bryn Mawr Hospital and received her MPH from UCONN Medical Center. Dr. Mabasa sees active listening and communication as keys to a successful doctor-patient relationship.
Our Standard of Care
You will be seen by your provider
Our providers value face time with their patients. The medical industry has seen a shift away from primary care provider continuity and interactions. We believe patients should be able to see their medical provider directly every time they come in for a visit.
Focused Care | Limited Populations
Our medical providers have more direct time to focus on you and your care. We accomplish this by continually managing and limiting our total population size to 1/5th the industry standard.
Extended one-on-one time with your primary care provider
We extend our slotted time with patients to allow more time one-on-one with your medical provider. We aim for visits to never feel rushed and to provide adequate time for proper exams and discussions with your provider.
Timely Appointments
We show respect to our patients and their time by making sure appointments start when they are scheduled.
Follow up calls from our office about your care
Our office engages in following up with patients to help remind, support and answer questions about your care. We follow up with whatever is needed to support your care from test results to reminders about outstanding orders made by your provider.
